CME Evaluation - Teaching Population Health - October 25, 2022

This session will start with an introduction to population health, including: the difference between public health and population health, defining a population or panel of patients, defining metrics for population health, and developing teams to manage panels of patients. The session will then cover educational concepts needed to teach population health management to all levels of learners including Undergraduate, Graduate and Continuing Medical Education (UME, GME and CME learners). The session will conclude with concrete tools that can be used to teach and give learners hands on experience with population health management including: Electronic Medical Records (EMR) dashboards, report cards, and in/outreach tools.

  1. Differentiate population health from public health
  2. Name at least 3 ways populations can be defined in healthcare
  3. Name 3 tools that can be used to teach population health
  4. Teach other learners to develop a population health team
