Maryland Chapter Meeting: October 3, 2023 - Integrating OUD treatment into Post-Acute Care: Hospital, SNF and OTP Partnerships
The opioid overdose crisis continues to claim many lives, currently driving by illicit synthetic opioids such as fentanyl. The medications buprenorphine and methadone have been shown in systematic review to decrease all-cause mortality. This activity will review evidence for hospital-initiated MOUD, identify barriers to integrating OUD treatment into post-acute care, and then describe model of successful hospital, opioid treatment program (OTP) and skilled nursing facility (SNF) partnership in order to help participants develop a plan to improve transitions of care for hospitalized patients with OUD in their practice settings.
